SHA ACTIVITY OF THE YEAR CONTEST ENTRY FORM

1. Select an official activity that your SHA Chapter has completed during the 2007-2008 school year.

2. Select ONE appropriate area in which to compete (see below).

3. Document your efforts. Compile you entry in notebook or scrapbook form with descriptions, photos, newspaper clippings, statements from members, teachers, administrators, and parents. Include anything you like that will show evidence of your efforts.
