Certificates for participation and service

Can somebody, please address me where can I find cerfitifcates for students that were active in the chapter during the school year. I remember seen a format somewhere, can somebody thell where can I find them. Thanks,

    There’s no form on the web site.. If you are looking for Certifcates of Service, you have to order them from SHH. You are entitled to 10% of the number of initiates for this year. The form to order them is in the Becas packet.
